This form is for capturing a prospective volunteer's information, skills, and preferences (we do not share your info outside PIF without your permission). To volunteer at PIF, you must be 18 or older or be accompanied by an adult for your first visit.

Volunteer Understanding and Agreement to Release Assumption of Risk and to Hold Harmless<br />

I, ____(Signee)_____am aware that there are certain inherent risks involved in participating with the Pedal It Forward NWA program. Risks include but are not limited to:<br />
<br />
- Personal injury from participating in on-site and off-site bike repair, assembly, testing, and other bike repair activities;<br />
- Personal injury from participating in rides and recreation activities including bike road testing;<br />
- Personal injury and property damage from transporting bikes and equipment; and,<br />
- Theft of or damage to my property.<br />

In consideration of my being granted permission to participate with Pedal It Forward, NWA, Inc. in their activities and to use the facilities of Pedal it Forward NWA and/or other services provided by Pedal It Forward NWA, its agents and employees, including training, riding, and other related activities, I, on behalf of myself, my executors, administrators, heirs, next of kin, and successors, hereby covenant to hold harmless and indemnify Pedal It Forward NWA, and all of their officers, agencies, agents, and employees from any and all claims for damages, injuries, fines, penalties, and costs (including court costs and attorney's fees), charges, liabilities, or exposures, however, caused, resulting from or arising out of or in any way connected to my participation in Pedal It Forward NWA programs.<br />

I understand that bike building and repair, riding, and other volunteer activities have a risk of injury.<br />
I have read and understood this agreement and by my signature agree to its terms. <br />
Pedal It Forward NWA provides no accident or medical insurance to volunteers or participants.<br />

Photography Release<br />
I give Pedal It Forward, NWA permission for any responsible use of photographs taken at Pedal It Forward, NWA workshop, property or events in which I appear, or in which my family member appears. I understand that the photographs may be used in displays, publications, or sent to the press for publication in a newspaper or use on television.

I am over 18 or the legal guardian of the participant above and I accept the terms of the waiver.
