NYCGHA Annual Membership
About This Registration
To participate in all NYC Gay Hockey Association on-ice and select social activities, you are required to become a dues-paying member. Please read the membership terms and conditions. On the next page, you will be asked to acknowledge that you agree to our code of conduct and concussion policy, fill out your member profile, and pay the membership fee. Note that your membership fee is 100% tax-deductible. Dues-paying members will be eligible to skate on NYCGHA teams, participate in exclusive member events and receive select perks that we will announce on our website and through social media.
TERMS AND CONDITIONS
You can participate in any NYC Gay Hockey Association on-ice activity once your membership dues are processed. This includes but is not limited to:
Playing on any NYCGHA team as a full-time skater, goalie, or alternate. This includes the Lions, Wizards, Boxers, and all teams in the NYCGHA Recreational League.
Participating in any NYCGHA scrimmage as a skater or goalie.
Playing with any NYCGHA-sanctioned teams in tournaments or leagues outside of Sky Rink.
Select social activities and events.
As a member of the NYC Gay Hockey Association, you must read and agree to the terms in our Code of Conduct and Concussion Agreement. Each can be found at:

You will need to acknowledge your receipt and understanding of the Code of Conduct and Concussion Agreement on the next page.
Notes About Survey Questions
All information collected for membership will only be shared with the the NYCGHA Board of Directors. Survey questions are presented for informational purposes only in order to gather statistics about our membership. All responses will remain anonymous except to the NYCGHA Board of Directors.
Additionally, contact information (phone number and email) will be provided to captains as well.
We do not provide any of your personal information to third-parties.
