Imagine a community where every resident has access to high-quality, affordable health care and wrap around services.
The story of the Primary Care Coalition began in 1993 when a group of physicians, activists, and public health officials envisioned just such a community. They pictured a dependable source of primary care for people with limited resources and no health insurance, and they created a framework that shared the responsibility for providing that care across the public and private sectors.
The Primary Care Coalition was formed to help make that vision a reality.
We've Come A Long Way.
Since our founding, the definition of "primary care" has expanded. We know that an integrated network of health services working in concert with primary care providers is necessary to ensure patients receive comprehensive care. Through innovative thinking, hard work, and dedication to our vision, the PCC and our partners worked together to develop a safety-net system of care that evolves with the needs of the population.
We've accomplished a lot in 25 years, but we have so much more to do. Your support as we celebrate this milestone will help make sure we start the next chapter of our journey with the necessary resources to continue to innovate and push for system-wide improvements that support our most vulnerable neighbors in living more healthy lives.
