I realize that participation in the FEIEF Oh, the Places You'll Run! 5K Run and 1K Fun Run may involve risk from physical activity and other causes. I consent to participate in this activity, and I assume this risk. I medically certify FEIEF that the above named person has no medical or physical infirmity that may render participation in the race inadvisable, are of suitable age to participate in the race, and will properly prepare for the race. I authorize FEIEF, its directors, employees, agents, sponsors, and volunteers authority, at any such persons discretion and at my expense, to administer or to procure of me any medical attention they may deem necessary. I agree and abide by decisions of all race officials. I release FEIEF, its directors, employees, agents, sponsors and volunteers from all clams or liabilities of any kind arising from my participation in this event. I grant permission to FEIEF to use any media, including pictures of myself/my child in this event, for publicity/promotion purposes this year and in future years. I also understand that no pets will be allowed at this event. My signature verifies that I understand and will abide by this legal wavier of liability.
